520-Invalid Command Syntax if choosing Operator on SAIL IVR Timeout

We have a greeting message saying "If you would like to speak to our reception, please hold, or press 1 if you know the extension number."

Every time we stay on hold, the call keeps disconnecting, and AGI debug says:

      520-Invalid command syntax. Proper usage follows:
      Changes the extension for continuation upon exiting the application.           
      <SIP/peer3350-00000082>AGI Tx >> 520 End of proper usage.
      SET CONTEXT extensions
      200 result=0
      AGI Script completed, returning 0
      Sent into invalid extension '61740123456' in context 'extensions' on SIP/peer3350-00000082
      Executing [i@extensions:1] PlayTones("SIP/peer3350-00000082","congestion") in new stack

Our IVR (e.g. Welcome) Timeout is pointing to Operator.
Our Operator Real Extension under Global settings is 4000.
But when I changed the Timeout of the IVR to point to 4000, the call directed straight to the operator's extension OK.
Which line of code do we have to check so asterisk can read the real extension of the Operator?

Not sure why this is happening but it does look like a bug.   
Just point to the real operator for now.
