Is it possible to create a user via the command-line interface?
If so, what is the recommended method?

You can add a user via server manager and then review the messages log file to see what commands are issued.

Also take a look in
/etc/e-smith/events
and
http://wiki.contribs.org/SME_Server:Documentation:Developers_Manual#Accounts
