Hi,
 I have an odd issue that I don't know where to begin to debug.

The problem is.
My parents are on the same VSP as one of my Asterisk trunks, so I can call them for free.
If I call them from an extn that has a Cisco IP phone, the call works but they cannot hear me at all.
If I call them from my general house phone xtn, which is via SPA3102 the call works without any issue at all.
Calling any other number from the Cisco IP phone xtn works without any issue.
At my parents end they use a Billion 7404 as their ADSL modem and VOIP ATA, they have no issues typically, only an annoying son that rings but doesn't speak to them.
I figure the issue has to be my end with the SAIL / asterisk server, but what could possibly cause this issue in such an odd combination ?

You'll need to install tethereal and sniff the call in order to see where the RTP flow is being lost.  My guess would be that it isn't asterisk/sail per se but it maybe in the Cisco sip entry or the actual phone definition.  Check CODECs and return addresses in the RTP and SIP packets.
