ping to external ip request time out only reply when restartwhy and how to solve

I work in sme 8.0
I work in server only mode without DHCP
My domain altawi.com.sa
Computer name mserver
Connected with Cisco firewall router behind back of server(Cisco 800)
and cisco router used as gateway in side sme mail server as IP 192.168.1.254
External IP is 78.93.244.61
this is my settings it work OK before and  send and receive but now not work
suddenly when i try to ping to external IP 78.93.244.61 it give me
request time out
i remove wire connected to router Cisco
then replaced with another wire the problem until now exist
the problem ping to external IP not reply and status of access internet is failed
i face strange problem
i ping to external IP 78.93.244.61(work before)it give me
request time out
i restart modem Cisco
it give me reply request(and i can send and receive emails) and after 5 second
 this back to request time out(and cannot send and receive emails)
only i receive reply when i restart modem Cisco
i have  lightes in modem connected and green light
cd green light and fixed signal not blank
ppp green light and fixed signal not blank
dsl light green work also
but not publish to external  IP
inside or internal my network i can send and receive emails but outside i cannot send or receive emails
What is the problem if possible help me AND How to solve it
Access internet to SME server is failed because is ping to outside or external IP is request time out
Why IP give me request time out

PING TO EXTERNAL IP 78.93.244.61
REQUEST TIME OUT
RESTART MODEM
REPLY GOOD(send and receive is OK)
AFTER 5 SECOND
BACK TO REQUEST TIME OUT(send and receive is not OK)

My settings is fixed before 5 years and not new

5) When did it stop working ? What things did you do or change immediately before "it stopped working" ?
ie what has happened to your settings, your equipment, your network layout, installed software (on workstations & server), Cisco router settings & hardware, ISP plan & connection etc etc etc, since the last time you were aware that "it worked OK" ?

6) Have you looked in log files on your server, workstation & in the gateway router ? Where you look at the log files will depend somewhat on where you are pinging from.

Please search before asking, an answer may already exist.

*yes sme fully updated
*i try to ping from workstation not from  mail server
*Only one Cisco router with gateway 192.168.1.254
*nothing happen OR changes suddenly cannot ping to external IP
nothing changes in equipment
i can ping or connect good to mail server locally
problem only when ping to external IP

We need to determine first the problem
then call ISP

well.. if restarting the router make things work just for few seconds, I guess something is wrong with your router or with your connection.

in both cases, you'd call the ISP and check everything is ok with them..
if you had problems in the past days (and IIRC you had), maybe your ISP blocked you (I've seen such things many times)

I cannot access internet on this ip
i call company isp
it tell me every thing work OK

since you are pinging your external IP on your cisco router from a workstation with fixed IP, can you tell me in which way SME is involved?

trying to ping 78.93.244.61 from here.. request time out..

told so, you:
- may have your router blocking incoming IMCP packets
- may have some network issues..

in any case, since SME is in server only and I'm quite sure that who'd echo to ping is the router, it's not a SME issue.

you say that ISP told you everything is working ok.. well, it's not
